Programme Summary

Understand unsafe outcomes by examining the conditions that shape work and human performance. Participants distinguish error types, explore task and organisational influences and practise constructive safety conversations. Workplace cases connect observations with practical redesign and reliable controls, helping teams learn from everyday work without reducing complex risks to blame, observation targets or assumptions that behaviour alone determines safety.

02

Training Objectives

1 Explain human performance within work systems; 2 Identify task and organisational error influences; 3 Use constructive observation and safety dialogue; 4 Prioritise error tolerant work and control design; 5 Evaluate behavioural programme limitations and outcomes.

03

Learning Outcomes

1 Analyse a task for error influencing conditions; 2 Distinguish slips lapses mistakes and violations; 3 Conduct a constructive safety observation conversation; 4 Propose practical work system improvements; 5 Define balanced measures for learning and control.

04

Target Audience

Safety Advisors; Operations Supervisors; Safety Culture Coordinators; Human Factors Coordinators; Maintenance Supervisors.

05

Target Industries

Manufacturing; Construction; Logistics and Warehousing; Facilities Management; Utilities; Hospitality Operations.

Course Contents

  • Module 1 Human Factors Framework: Job organisation and individual influences; Human capability limits and normal variability; Safety outcomes as system interactions.
  • Module 2 Understanding Human Failure: Slips lapses mistakes and intentional deviations; Error provoking conditions and conflicting goals; Limits of labels and retrospective judgement.
  • Module 3 Task and Work Design: Instructions interfaces workload and environment; Communication handovers and interruptions; Error tolerant design and recovery opportunities.
  • Module 4 Observation with Context: Selecting meaningful tasks and conditions; Listening to how work is actually done; Recognising controls beyond visible behaviour.
  • Module 5 Constructive Safety Conversations: Open questions respectful feedback and curiosity; Psychological safety and reporting trust; Agreeing actions without blame or coercion.
  • Module 6 Improvement Selection: Removing barriers to safe performance; Engineering organisational and procedural measures; When training or reminders are insufficient.
  • Module 7 Programme Evaluation: Quality of learning and action completion; Control effectiveness and unintended incentives; Avoiding observation counts as sole success measures.
  • Module 8 Human Factors Workshop: Analysing a realistic task failure case; Practising a constructive observation conversation; Presenting system improvements and review measures.
07

Assessment

  • 40 single best answer questions, including applied scenarios; one mark per question.
  • 60 minute invigilated digital examination with identity verification. Approved accessibility adjustments preserve the assessed competence.
  • Supplied reference pack and calculator permitted; external internet and AI prohibited.
  • Individual applied assessment (45 minutes): Analyse a task failure scenario, conduct an observed safety conversation and propose work system improvements with balanced effectiveness measures.
  • Assessment criteria: Contextual analysis beyond individual blame; Constructive evidence seeking communication; Effective system focused improvements.
  • Pass each component separately: 30/40 written marks (75%) and at least 70% on the applied task.
  • Individual results and feedback are provided. One reassessment may follow coaching under the published enrolment terms.
08

Certification

  • Successful learners receive a TQCSI Arabia digital Certificate of Successful Completion.
  • The certificate records the learner name, exact programme title, teaching hours, completion date and unique verification reference.
  • Verified identity, full attendance and separate written and applied passes are required; all mandatory critical criteria must be met.
  • Proposed issue target: within 30 calendar days of approved results and settlement of applicable fees.
  • Records behavioural safety and human factors learning; it does not confer specialist human reliability analysis certification.
  • Attendance confirmation may record completed hours for unsuccessful learners; it does not certify successful completion.
