Programme Summary
Resolve recurring quality problems through a disciplined eight stage investigation. Participants establish containment, develop a precise problem description and verify both occurrence and escape causes before selecting permanent actions. A supplier quality case develops an evidence based report, validation plan and prevention review, helping teams communicate clearly with customers and avoid closing issues before controls have proved effective.
Training Objectives
1 Structure disciplined cross functional problem resolution; 2 Protect customers through effective temporary containment; 3 Verify occurrence and escape causes separately; 4 Validate permanent corrective actions before closure; 5 Strengthen prevention and customer communication evidence.
Learning Outcomes
1 Prepare a precise quality problem description; 2 Define containment and effectiveness checks; 3 Verify occurrence and escape cause hypotheses; 4 Develop a validated corrective action plan; 5 Write a complete evidence based 8D report.
Target Audience
Quality Engineers; Process Engineers; Quality Managers; Production Supervisors; Continuous Improvement Specialists.
Target Industries
Automotive Manufacturing; Electronics Manufacturing; Industrial Equipment Manufacturing; Medical Device Manufacturing; Consumer Goods Manufacturing; Industrial Services.
Course Contents
- Module 1 Problem Response and Team Formation: Issue severity and customer protection; Cross functional roles and technical expertise; Scope ownership and response timing.
- Module 2 Problem Description: What where when and extent; Is and is not comparisons; Evidence sources and baseline definition.
- Module 3 Temporary Containment: Suspect product scope and segregation; Detection method suitability and verification; Customer communication and containment monitoring.
- Module 4 Occurrence Cause Investigation: Process changes and causal hypotheses; Data based testing and technical evidence; Separating contributing factors from root causes.
- Module 5 Escape Cause Investigation: Why existing controls missed defects; Inspection measurement and reaction weaknesses; Verifying the detection system failure.
- Module 6 Permanent Corrective Actions: Cause linked action selection; Risk review and trial validation; Demonstrating effectiveness before full release.
- Module 7 Prevention and Closure: Updating standards controls and training; Applying learning to similar processes; Removal of containment and closure evidence.
- Module 8 8D Report Workshop: Integrating evidence into a concise report; Challenging unsupported causes and actions; Presenting verification and prevention decisions.
Assessment
- 40 single best answer questions, including applied scenarios; one mark per question.
- 60 minute invigilated digital examination with identity verification. Approved accessibility adjustments preserve the assessed competence.
- Supplied reference pack and calculator permitted; external internet and AI prohibited.
- Individual applied assessment (30 minutes): Complete an individual 8D report for a supplied defect case, including containment, verified cause reasoning and an action validation plan.
- Assessment criteria: Precise problem and containment logic; Evidence supporting occurrence and escape causes; Validated actions and prevention coverage.
- Pass each component separately: 30/40 written marks (75%) and at least 70% on the applied task.
- Individual results and feedback are provided. One reassessment may follow coaching under the published enrolment terms.
Certification
- Successful learners receive a TQCSI Arabia digital Certificate of Successful Completion.
- The certificate records the learner name, exact programme title, teaching hours, completion date and unique verification reference.
- Verified identity, full attendance and separate written and applied passes are required; all mandatory critical criteria must be met.
- Proposed issue target: within 30 calendar days of approved results and settlement of applicable fees.
- Records structured 8D problem solving skills; customer acceptance and product disposition require the organisation’s authorised decisions.
- Attendance confirmation may record completed hours for unsuccessful learners; it does not certify successful completion.
